Bayhealth Recognizes Social Workers

Bayhealth is recognizing its social workers in March during National Professional Social Work Month. This year’s theme, “All People Matter,” seeks to showcase the impact of the dedicated social workers at Bayhealth as well as the more than 600,000 professional social workers nationwide.

From finding durable medical equipment for a pediatric patient, to identifying a skilled nursing home for geriatric patients, Bayhealth social workers help patients throughout the entire continuum of care. 

Bayhealth’s social workers play a crucial role in ensuring patients receive needed resources during and after their hospitalization by partnering with a team of physicians, nurses, pharmacists, rehab therapists, nutritionists and family members to develop a comprehensive discharge plan for each patient. Social workers play a vital role in arranging services to bridge the transition to the next level of care, whether it is to another facility or their home.

In addition, social workers may assist with negotiating the complex world of Medicaid, Medicare, and other insurance plans.

“We appreciate the dedication and commitment our Social Workers have in partnering with their colleagues and community resources to make safe, effective discharges for our patients” says Dr. Eric Gloss, VP of Medical Affairs at Bayhealth. “They are an integral part of Bayhealth’s efforts to implement evidence-based practices, reduce gaps in care and improve transitions of care.”
