WHAT INFORMATION DO WE COLLECT
We automatically collect the domain name you use to access the internet (for example, aol.com, if you are using an America Online account), your IP (internet protocol) address and associated host names, date and time of your visit, the pages you visited, and the address you were visiting immediately prior to accessing Bayhealth's web site. The information we automatically collect on our visitors is aggregated and used solely to determine the number of visitors to our site and the sources of traffic.
We do not automatically collect your e-mail address. The only way we obtain your name or e-mail address is when you voluntarily provide us with such information. Examples of how you might provide us with such information include:
Completing a survey or feedback form;
E-mailing us with a comment or question;
Submitting a job application;
Registering for a class or event;
Submitting information to Support Groups Online and Ask the Educator;
Submitting information to the Physician Referral Service; or

HOW DO WE USE THE INFORMATION WE COLLECT
Bayhealth does not send unsolicited e-mail messages to visitors. Bayhealth may contact visitors by e-mail for purposes of responding to questions, comments, surveys or feedback forms. In addition, Bayhealth may send e-mails to visitors that are subscribed to an e-mail bulletin.
If a web site visitor requests materials that cannot be transmitted electronically, we will request the visitor's name, e-mail address, telephone number and mailing address, so that the materials can be sent U.S. mail. Such information is destroyed upon fulfilling the individual's request.
Bayhealth will use and disclose visitors' individually identifiable health information obtained from this web site in accordance with the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and its implementing regulations.
HOW CAN I GET MY NAME REMOVED FROM YOUR FILES
Send the Web Manager an e-mail at Jim_Welsh@Bayhealth.org that includes your full name, active e-mail address, and a request to remove your information from our files. We will process your request as quickly as possible.
This web site is not designed to attract children under the age of thirteen (13). Currently, Bayhealth does not knowingly collect any individually identifiable information from children under the age of thirteen (13). Bayhealth will comply with the Federal Children's Online Privacy Protection Act of 1998 ("COPPA"), if Bayhealth decides to collect any individually identifiable information from children under the age of thirteen (13).

BAYHEALTH'S INTERNAL SECURITY & PRIVACY PRACTICES
Bayhealth strives to operate a secure data network protected by password protection. Our security and privacy policies are periodically reviewed and enhanced as necessary. Only a limited number of Bayhealth employees have access to the information provided by our web site visitors. Bayhealth has technology that helps monitor, identify and prevent the unauthorized access, use, and/or vandalism of our web site and network computer systems. Despite these security measures, you should be aware that unauthorized third parties may attempt to intercept or access the information contained on our web site.
