Care Coordination
Bayhealth’s oncology care coordinators help patients through the entire oncology journey from diagnosis to treatment and serve as valuable resources even after treatment has been completed. These nurses and social workers also help with resources such as pharmaceutical, transportation, financial, insurance, home health, hospice and appointments with specialists when needed.
Nutrition Services
Every patient receiving oncology services at Bayhealth is screened for nutritional risk. Patients who have known risks are referred immediately to a registered dietitian for help. And throughout the treatment course, each patient is re-screened so that any nutrition-related concerns can be identified and addressed before problems develop.
