Celebrating Case Management Week


Bayhealth celebrates the contributions of its Care Management Department during Case Management Week, October 14 – 20, 2012. The American Case Management Association sponsors the week to educate the public about how care management professionals are a vital resource for hospital patients and their families.

The theme of this week’s celebration is "Patient-Oriented, Outcome Driven.” Bayhealth’s Care Management team is lead by Care Management Director Joanne Fletcher, RN, MSN, with assistance from Clinical Manager Tasheema Galarza, RN, MSN; Social Services Manager Lynn Davis, MSW; and Office Operations Supervisor Sharon Coleman. The department also includes 10 full-time Social Worker/Discharge Planners, eight relief Social Worker/Discharge Planners, 18 full-time Care Managers, 4 Clinical Documentation Specialists one relief Care Manager, a Recovery Analyst and two Secretaries.

The Bayhealth Care Management team helps patients understand their health status and treatment options. Care Management coordinates the patient’s spectrum of post-hospital care from physicians to rehabilitation centers to nursing homes. The Care Management staff works closely with physicians and members of a multidisciplinary team to ensure that services during a hospital stay are of the highest quality, are appropriate to patients’ needs and are provided in a timely manner.

“Patients and their families face difficult challenges when they are hospitalized. We want them to know that we are here to support them, provide information and advocate for their needs,” said Care Management Director Joanne Fletcher.
