These reports were created and are posted in accordance with the community health needs assessment requirements (CHNA) added by the Affordable Care Act and codified as part of the Internal Revenue Code requirements of section 501(r)(3).
We recognize that health equity is the state in which everyone has a fair and just opportunity to attain their highest level of health. We are committed to developing sustainable efforts to transform our community’s health. We are implementing state-of-the-market Epic technology tools that help reduce any potential healthcare disparities based on race, ethnicity, sexual orientation, gender identity or other demographic characteristics. Those solutions include interactive dashboards enabling analyses of preventative care, maternal outcomes, patient satisfaction and the social determinants of health, to name a few. We are committed to equitable care for our community and have targeted specific outreach to communities and patients in need based on data.
