Community Health Needs Assessment

As a not-for-profit health system, Bayhealth is committed to fulfilling the organization’s mission “to strengthen the health of our community, one life at a time.”  We honor our tax-exempt status and our responsibility to the communities we serve through various programs, activities and partnerships that are aimed at improving the health of our communities.

The details of our Community Health Needs Assessment (CHNA) can be found in the information below.  These reports were created and are posted in accordance with the community health needs assessment requirements (CHNA) added by the Affordable Care Act and codified as part of the Internal Revenue Code requirements of section 501(r)(3).

We recognize that health equity is the state in which everyone has a fair and just opportunity to attain their highest level of health. We are committed to developing sustainable efforts to transform our community’s health. We are implementing state-of-the-market Epic technology tools that help reduce any potential healthcare disparities based on race, ethnicity, sexual orientation, gender identity or other demographic characteristics. Those solutions include interactive dashboards enabling analyses of preventative care, maternal outcomes, patient satisfaction and the social determinants of health, to name a few. We are committed to equitable care for our community and have targeted specific outreach to communities and patients in need based on data.
