Community Health Needs Assessment

We care about your health. And every day we work to strengthen the health of our community, one life at a time.

We’re asking you to take a short survey to help us in our mission. Your answers will tell us how we can meet the healthcare needs of you and your family—and our entire community. Plus, how we’re currently doing in this area.

You will have the chance to enter a raffle for a $100 Visa gift card at the end of the survey as a thank you for taking it.

We’re required to do this Community Health Needs Assessment (CHNA) as a part of the Affordable Care Act.

The survey must be completed by April 30, 2022.

There are two different surveys: One for the community, including our patients and others living in central and southern Delaware. And one for stakeholders, including our physicians, schools, businesses, organizations, churches, and local and state government agencies.

Past CHNA Reports

As a not-for-profit health system, Bayhealth is committed to fulfilling the organization’s mission “to strengthen the health of our community, one life at a time.”  We honor our tax-exempt status and our responsibility to the communities we serve through various programs, activities and partnerships that are aimed at improving the health of our communities.

The details of our past Community Health Needs Assessment (CHNA) can be found in the information below.  These reports were created and are posted in accordance with the community health needs assessment requirements (CHNA) added by the Affordable Care Act and codified as part of the Internal Revenue Code requirements of section 501(r)(3).
